GSDC of Wisconsin - Membership Application

• You may either apply online here or print out a paper application (one per family member please) and mail it with your check to our Membership Chair
• Applicants must be eighteen (18) years of age or over.
• Family memberships must fill out the Family portion of the application online or if using a paper application, each applicant must fill out an application blank.
• All dogs must be AKC Registered German Shepherd dogs or have an ILP or PAL number.
• All dogs must be current with all vaccinations as required by the State of Wisconsin and recommended by the owners' veterinarian.
• Annual Dues are $78 per year for single memberships or $96 per year for family memberships.
• Training Dues are $100 for the first year (plus a $10 initiation fee per person) and thereafter are $50 every 6 months.
• A check for membership & training ($138 for a single membership - $166 for a family membership) must be received by the Membership Chair in order for an application to be processed.
